Chair Massage Agreement

By submitting my consent below, I am agreeing to receive a complimentary chair massage offered by Anthony Medical & Chiropractic Center. I understand that:

1. I am under no obligation for further services,
2. I have no financial responsibility for the complimentary chair massage services provided, and,
3. I understand the following advisements:
   1. I give my permission to receive a complimentary chair massage.
   2. I have no medical or physical ailments that prevent me from receiving a chair massage.
   3. I understand this massage is not meant to resolve pain or as a medical therapy.
   4. I understand the massage therapist does not diagnose illnesses or injuries.
   5. Though rarely experienced, I understand that after receiving a chair massage, I may notice:
      • Superficial bruising
      • Short-term muscle soreness
      • Exacerbation of undiscovered injury
   6. I understand it is my responsibility to inform my massage therapist of any discomfort I may feel during the massage session so he or she can adjust their technique accordingly.
   7. I understand that I can terminate the session at any time.
   8. I understand I must pay close attention when sitting on the massage chair and standing safely after receiving the massage.
   9. I release the company and massage therapist from all liability concerning any injuries that may occur during the massage session.
